- On the lines labeled "Print Name Here" type in your name(s) as they appear on the stock certificate.
- If you purchased stock in the name of someone who is currently a minor, please type their name as it appears on the stock certificate on the line(s) labeled "Print Name Here". Click the "Send" button and the proxy
- If you have had an address change since the last mailing, please type in your new address at the bottom of the proxy.
